Usually getting the dominant hand too involved in the swing can cause this so for right handed players make sure to keep the right hand passive through the swing for best results.

Focus on pulling the club with the left hand through the hitting zone as this will encourage an inside to out swing path that will help to promote straighter shots. Also make sure to swing within yourself, it is fine to swing hard as long as you know you can maintain good balance and control.
